Nurse's Office

Greenwich Catholic School requires a number of health forms to be completed and returned to the Nurse's Office. Please find the required forms below and click on the file name to download. If you have any questions, please contact Mrs. Garabedian in the Nurse's Office. 
It is important to follow the guidelines for childhood illness to ensure the health and safety of all students. If your child is feeling unwell, please assess their symptoms, consult the attached guidelines, and keep them home from school. Children should be symptom-free for at least 24 hours without medication before returning to school. Following these guidelines helps prevent the spread of illness and ensures a quicker recovery for your child.

If your child requires a prescription or any over-the-counter medication during the school day, you must follow the procedures required by Greenwich Public Schools, CT General Statutes, Sec. 10-212a, and CT Administrative Regulations, Sec. 10-212a-1 through 10-212a-7. These procedures promote safe practices for student and staff. Please read them carefully. No over the counter medicines, even cough drops, should be carried by a student in school unless the nurse is aware and the procedure below has been followed.  Please download and return the forms below to the Nurse's Office.

SPORTS FORM
 
Greenwich Catholic School children in Grades 5-8 are able to participate in Intramural and Interscholastic Team Sports.

As per the Town of Greenwich School Health Services recommendations and the Diocese of Bridgeport mandates, Greenwich Catholic School will require an annual Sports Physical Examination Form completed for all students Grades 5 – Grade 8 enrolled in all Interscholastic Varsity & JV Sports teams.
