Dear Parents, 

*Enclosure: Sports Physical Exam Form 

        Grades 5-8 

Greenwich Catholic School children in Grades 5-8 are able to participate in Intramural and Interscholastic Team Sports.

Students will not be able to participate or tryout unless all health requirements are fulfilled. 

 

As per the Town of Greenwich School Health Services recommendations and the Diocese of Bridgeport mandates, Greenwich Catholic School will require an annual Sports Physical Examination Form completed for all students Grades 5 – Grade 8 enrolled in all Cohort, Interscholastic Varsity & JV Sports Teams. 

Physicals must be dated within thirteen (13) months prior to the starting date of tryouts for 

that sport team or after school programs. 

We are enclosing a Sports Physical Examination Form to be completed by the student’s physician and parents. The Sports Permission Form and Permission for Medical Treatment Form will be distributed in September for parent completion. 

We recommend that you now schedule with your child’s physician an appointment for a 

summer examination to prevent appointment delays due to busy physician schedules. 

Students will not be able to try out or participate unless the above requirements are fulfilled. 

Please return completed Sports Physical Forms to the school for Mrs. Garabedian, RN 

to review for Fall Sports participation.
